Intermediate Care Homes (ICF/ID)

These homes are for those individuals with a variety of needs, who require specialized care. Park View and Courage homes are in Sioux City, IA and Bluff View is in Sergeant Bluff, IA.

Supervised Community Living (SCL)

Is an option for those individuals who can reside independently with minimal support. Current SCL programs are in Cherokee, IA, and Pinnacle Apartments and Spring Hill Apartments in Sioux City, IA.

Home and Community Based Services (HCBS)

Provides individuals with an opportunity to live cooperatively with others in a neighborhood setting. Staff members are always available to encourage responsible choices. HCBS locations include Cherry House in Cherokee, IA; Lawton Apartments in Lawton, IA; and Cheyenne House, Leech, Maple View, Marshall Apartments, South Maple Apartments, South Mulberry, South Royce Apartments, South Rustin Apartments, South Westcott, Spring Hills Duplex, St. Mary’s Apartments, and Stone Ridge Apartments in Sioux City, IA.

Medically Fragile

This home is for those individuals who are medically fragile.
